I went to visit a friend being rehabilitated here on two occasions.
Staff may be friendly and nice but they let a patient open an emergency exit door to smoke a cigarette, allowing the smoke to waft into the hallway as the alarm went off. No one told her to stop, go completely out or close the door.
If you have mobility issues, they have steps in the "front" and the driveway is uneven and rutty. Food luckbtrying to get anyone's attention to get access to get inside.
When I visited my friend the second time, she looked much better and also was more lucid.

This rating is generous basesd on these 3 breif interactions over the course of 2 hours. I previewed this place before.my mom was admitted there and I saw what I thought to be a guest and it turned out to be a employee playing loud music in the parking lot, which os located right next to the dinning room. We also found a diaper in the drawer next to my moms bed. Lastly, the TV scren in the room, was shared with 2 people was no larger than a 15in laptop.

Lake Merritt is at the site previously owned by ( and still has the sign up that says ) oak grove. Its an old facility with friendly nursing staff that do actually care for their residents. What they lack in appearance they gain in the team family feel. The PT/OT team is getting my grandmother stronger and the administrative team has kept their word by getting my grandmother moved to a private room (which they only have one of). Food is not that great. They claim to be planning to do upgrades to the place in the near future and even though they are OK for my grandmother's short stay given our unique circumstances, they definitely have room for improvement.

It went from OK to awful in my grandmother's total of 6 month stay. The moment her 100 medicare days were up her care changed drastically. PT/OT stopped and they kept her in a wheelchair banning her from her Walker. Her mobility DECLINED from being in their care. Discharge was a disaster and so unorganized. DON never available and constantly passed the buck instead of dealing with complaints. Had to get the ombudsman involved during her last 2 months.
